Forensic examination of the corpse

November 22, 1951. Based on the proposal of the head of the medical unit of prison No. 1, Captain Rostov, from 22/XI–51, in the prison hospital, in cloudy weather, in natural daylight, in the presence of hospital doctors MIRUTS M.A. and NEVSTRUEVA(?) V.M.(?), I, forensic expert KUNITSKAYA N.N., conducted a forensic medical examination of the body of prisoner Yachmenev Venidikt Spiridonovich, born in 1895, to determine the operative cause of his death.

Preliminary information on the circumstances of death. According to the medical history, prisoner Yachmenev V.S. was admitted to the hospital of prison No. 1 on 10/11–51 with a preliminary diagnosis: astrophic cirrhosis of the liver /ascites/.

Final diagnosis: atrophic liver cirrhosis/myocardial dystrophy/degeneration. Circulatory disorder of the 3rd degree. Associated diseases: left-sided exudative pleurisy. Died on 21/11–51.

A. External examination.

The body of a man, approximately 55 years old in appearance, of normal build, with severe undernutrition. The length of the body is 178 cm. Cadaveric spots of a pale purple color are located on the back of the body, reaching the mid-axillary line, do not fade or disappear when pressed with a finger. Rigor mortis is weakly expressed. There is no putrefaction. The eyes are open, the corneas are cloudy, the pupils are evenly dilated. The external auditory canals and the openings of the mouth and nose are free. There are 8 teeth in the mouth, the tongue is behind the teeth. The bones of the face and the cartilage of the nose, as well as the long bones of the limbs are intact to the touch. On the forehead and temple on the left there are small abrasions of parchment density, covered with a reddish crust. The skin is an earthy color. The visible mucous membranes and subungual spaces of the fingers are bluish. The chest is cylindrical. The abdomen is in a changed configuration /frog abdomen/, slightly below the chest level. Below the navel along the midline there is a trace of a needle puncture, covered with a gauze sticker. The external genitalia are developed correctly. The anus is closed, clean in circumference. No other changes or damage were found during external examination.

B. Internal examination.

The subcutaneous fat of the anterior abdominal wall is almost absent. The arrangement of the organs of the thoracic and abdominal cavity is anatomically correct. Large cavities of the corpse are without foreign odor. The peritoneum is moist, smooth, grayish in color, the vessels are not sharply dilated. In the abdominal cavity there are about two and a half liters of turbid reddish-yellow liquid. The hyoid bone and cartilages of the larynx are intact, soft tissues are without bruises. The lungs on both sides are fused with the rib cage along their entire length. The pericardium contains a small amount of transparent yellowish liquid. The heart is 12x10 cm in size, flabby, moderately coated with fat along the furrows on the surface, in its cavities there are yellow dense clots of fibrin and loosely coagulated dark blood. The valves of the heart, aorta and pulmonary artery are translucent, flaccid. The heart muscle has a clay tint on the section. The coronary vessels freely pass the probe. The thickness of the wall of the left stomach is 1.2 cm, the right 0.4 cm. The aorta is 5 cm wide, the inner lining has a small number of small whitish plaques on its surface, and loosely clotted blood in the lumen.

ACT on the death of a prisoner

November 21, 1951, Tomsk

I, the head of the medical unit of prison No. 1 of the city of Tomsk, captain of medical service ROSTOVYKH G. V., and the attending physician, captain of medical service MIRUTS M. A., examined the body of prisoner of prison No. 1 of the Tomsk Regional Directorate of the Ministry of Internal Affairs, Venidikt Spiridonovich YACHMENEV, born in 1895, who died in the prison hospital on November 21, 1951 at 1:30 am.

Upon external examination, the following was found:

1. The corpse is male, of normal build, severely undernourished, visible mucous membranes and skin are pale. Above the left eyebrow there are two small abrasions, at the outer corner of the right eyebrow and on the right temple there are also small abrasions. On the right shoulder there are two large abrasions, on the chest and right thigh and knee there are also abrasions.

Rigor mortis is expressed in the extremities. On the underlying surfaces of the body, arms and buttocks there are cadaveric spots.

2. 10/XI – 51 YACHMENEV was delivered to the prison hospital from the internal prison of the UMGB in a serious condition for periodic abdominal puncture due to ascites. On the same day, the puncture was done, as a result of which 16.5 liters of transudate came out. The patient's condition improved somewhat, on November 12, according to the agreement, the patient was taken back. 13/XI – 51 YACHMENEV was admitted to the prison hospital for permanent detention. The patient's condition was serious, as indicated in the act drawn up upon admission.

The patient is bedridden, severely exhausted, with shortness of breath, ascites, swelling of the legs, congestion in the lungs, muffled heart sounds.

Abdominal puncture on 10/11/51 and symptomatic treatment somewhat improved the patient's condition, but not for long. On 20/11/51, abdominal puncture was performed again and 12 liters of hemorrhagic transudate were removed; the patient's condition is very serious.

On November 20, 1951, at 9 p.m. and at 11 p.m., the duty paramedic NOSKOVA visited the patient; the latter was in a half-asleep, calm state.

At 00:30 the hospital duty officer called NOSKOVA to the patient’s room, as he saw through the peephole that YACHMENEV had fallen near the bed.

Paramedic NOSKOVA, together with another patient prisoner, laid the patient on the bed; YACHMENEV was conscious and talked to her, but his condition was already grave – agony had begun.

At 1:30 am on November 21, aged 51, the patient died from increasing cardiac weakness.

Diagnosis: Atrophic liver cirrhosis. Heart failure grade III, left-sided exudative pleurisy.
